Referrals to Momentum

The listed referrals are some of the initiatives supported by the Australian Government, which for the eligible participant provide part or full benefit payments for our services. We accept all eligible referrals from the following services.

  • As NDIS providers, we are able to provide exercise physiology services to NDIS participants who are plan-managed, or NDIA managed.

    Exercise Physiology can be provided to clients with the following approved funding categories:

    Health & Well-being

    Improved Daily Living

    Therapeutic Supports

    Core Supports

    All new referrals will be able to obtain an appointment within 7 days.

    Please provide as much information as possible, so that we can fulfil your request appropriately. We endeavour to respond to your query within 24 hours.

  • These forms must be completed by your general practitioner under a Chronic Disease Management Plan to be eligible for the Medicare benefit.

    Note - We Bulk Bill ALL eligible Medicare claims for no out of pocket expense to patients

  • Eligible Veterans with a White or Gold Card are able to use this referral form when completed by their GP to access services funded by the Department of Veterans Affairs for recognised conditions.
